In recognition of the possible dangers connected with any physical activity, Members, visitors, and persons touring the facility hereby knowingly and voluntarily waive any right of cause of action of any kind whatsoever arising as the result of such activity from which liability may or could accrue to La Plata Fitness Clubs, LLC, Officers, Agents, Employees, Or Instructors.

Signing this registration form also serves as a conformation that you will abide by gym rules and policies, which are posted and available at the service desk. Also, that you agree to comply with penalties should you violate those policies.
